Presentation

Follow up case of tubercular abscesses in the brain.

Patient Data

Age: 40 years
Gender: Male

Axial post-contrast images show multiple anomalous variants of the aortic arch:

1. Right and left CCA are seen arising from aortic arch as a common trunk.

2. Right subclavian artery is seen arising from junction of aortic arch and descending aorta coursing in superior mediastinum posterior to the esophagus - aberrant right subclavian artery.

3. Right vertebral artery is seen arising from the right CCA.

4. Left vertebral artery is seen arising from aortic arch proximal to the left subclavian artery.

Case Discussion

An aberrant right subclavian artery can sometimes give rise to dysphagia by compression on the posterior part of esophagus which is termed as "dysphagia lusoria". It can also be a site for subclavian artery stenosis leading to vascular symptoms.
