Multiple exostosis

Case contributed by Maulik S Patel
Diagnosis certain

Presentation

Swelling at knee and arm. No local pain. Knee swelling has gradually increased during last 2 years.

Patient Data

Age: 20 years
Gender: Female
x-ray

Fractured exostosis of the humerus.

x-ray

Femoral and fibular lesions are present.

The tibial lesion is approximately 6 x 5 cm in size.

ultrasound

Thin cartilage cap in the femoral lesion. Most of the tibial lesion shows a thin cap.

The posterior-medial-cranial part of the tibial lesion shows cartilage cap thickness up to 18 mm.

Post surgical resection

x-ray

The large upper medial tibial lesion was excised surgically.

Case Discussion

An excisional biopsy of the tibial lesion was performed due to the risk of malignancy in a thickened cartilaginous cap (>15 mm). However, this was benign on histopathology.
