Multiple myeloma

Diagnosis probable

Presentation

Inability to walk, severe low back pain.

Patient Data

Age: 60 years
Gender: Male

There are innumerable lytic lesions involving all the visualized vertebral bodies, posterior elements, sacrum, pelvic bones and the ribs.

Degenerative changes are also noted in the lumbar spine.

Case Discussion

CT findings suggest multiple myeloma. The biochemical results show reversal of albumin/globulin ratio, Bence Jones proteinuria, and decreased alkaline phosphatase levels suggesting multiple myeloma.

The main differential diagnosis is diffuse lytic bone metastasis though metastasis commonly affects the pedicles more than the vertebral bodies.
