Presentation

Left wrist pain and swelling started 2 years ago. Right shoulder pain started 8 months ago. Presented with primary concern for back pain, restricting walking ability.

Patient Data

Age: 75 years
Gender: Male

Expansile lytic lesion of the scapula (body, acromion and coracoid process) with an associated soft tissue mass.

Expansile lytic lesions are present in the distal humerus and radius. The radial lesion has an associated soft tissue mass. A pathological fracture of the radius, with angulation, and dislocation of the distal ulnar has occurred.

Case Discussion

This patient presented with advanced disease, having had symptoms for 2 years. He was from a low-income country with limited medical facilities available in his region.

Although radial involvement was extensive, he still had use of his left hand, albeit with slightly reduced power compared to the right hand.

Biopsy of the soft tissue mass was performed under ultrasound guidance, with pathology report concluding plasma cell neoplasm. Due to further bony involvement (i.e. left shoulder, left distal humerus and left radius) with a similar process, multiple myeloma was the final diagnosis.
