Presentation

The patient presented with headaches for months. A history of breast cancer guided the oncologist to order a skull x-ray to rule out any lytic lesions.

Patient Data

Age: 70 years
Gender: Female
x-ray

Multiple, well-defined punched-out lytic lesions of various sizes are scattered throughout the skull (raindrop skull).

Case Discussion

  • the patient has normocytic normochromic anemia and the peripheral blood smear shows Rouleaux formation

  • serum free light chain assay shows increased free kappa chain

  • multiple myeloma confirmed by bone marrow biopsy
