Presentation

Lower back pain. Known case of multiple myeloma from 6 years ago.

Patient Data

Age: 85 years
Gender: Male

MRI of the lumbar spine demonstrates extensive patchy regions of bone marrow replacement due to infiltrative disorders. 

Spondylotic canal stenosis is present at the L2/L3 level in the background of disco-osteophyte bulging with thecal sac indentation and foraminal narrowing as well as ligamentum flavum hypertrophy.

Asymmetrical disco-osteophyte bulging with thecal sac indentation and right foraminal narrowing is seen at L4/L5 level.

Case Discussion

Multiple myeloma is a multifocal proliferation of plasma cells based in the bone marrow. It is the most common primary malignant bone neoplasm in adults. It arises from red marrow due to the monoclonal proliferation of plasma cells and manifests in a wide range of radiographic abnormalities.
