Multiple uterine leiomyomas

Presentation

Vaginal bleeding in a patient with primary infertility.

Patient Data

Age: 40 years
Gender: Female

The uterus is diffusely enlarged anteverted retroverted with multiple leiomyomas of various sizes and locations (Figo 1 to 6), the largest leiomyoma is of subserosal location (Figo 6) measuring 18 x 16 x 9 cm. They display an isosignal to the myometrium on T1, low signal on T2 with moderate heterogeneous enhancement on postcontrast sequences. The largest leiomyoma shows central areas of cystic degeneration/necrosis. 

Both ovaries appear normal with small follicles.
