Massive splenomegaly in a 72 year old with bony sclerosis is most likely myelofibrosis.
Early hepatomegaly as suggested by slight rouding of the inferior end of the liver.
DDx for splenomegaly of this size include malaria, Leishmaniasis, storage disorders and Thalasemia
