Myxoid liposarcoma

Presentation

Painful swelling of the superomedial aspect of the left thigh.

Patient Data

Age: 35 years
Gender: Female
mri

Large relatively well-circumscribed intramuscular mass of the superomedial aspect of the left thigh, centered on the adductor magnus muscle with no interface with the adjacent muscles It elicits a low signal with areas of slightly high signal to the muscles on T1, very high T2 with hypointense internal septae and areas of restricted diffusion. The post-contrast dynamic sequences show a progressive and heterogeneous enhancement with central areas of necrosis and thick enhanced septae. The sciatic nerve is compressed and displaced posteriorly with partial encasement of the femoral artery.

Normal thickness and signal intensity of the femoral cortex with homogeneous bone marrow.

Case Discussion

MRI features of a large intramuscular soft tissue mass centered on the adductor magnus muscle, histologically confirmed as a myxoid liposarcoma.

Myxoid liposarcoma is considered the second most common subtype of liposarcoma. It occurs most commonly in the lower extremity, particularly the thigh as in this case.
