Myxoid liposarcoma - soleus muscle

Presentation

Progressive painless swelling of the right calf.

Patient Data

Age: 40 years
Gender: Male

Large well-circumscribed lobulated soft tissue mass centered on the soleus muscle of low signal on T1 with small areas of high signal, high signal on T2 with hypointense internal septa. The post-contrast sequences show intense and heterogeneous  enhancement with no evidence of restricted diffusion the mean ADC was at 1.94 x 10-3 mm2/s.  This soft tissue mass appears to be in contact with the fibular cortex which shows normal thickness and signal intensity as well as homogeneous bone marrow. The neurovascular bundle is displaced anteriorly and gastrocnemius medial head anteromedially.  No involvement of the other muscles of the posterior compartment.

MRI features of a large well-circumscribed enhancing intramuscular soft tissue mass, pathologically proven as myxoid liposarcoma.

Myxoid liposarcoma is the second most common subtype of liposarcoma (after well-differentiated liposarcoma), representing 30-40% of all liposarcomas in the extremities. It most commonly occurs in the lower extremity, particularly the thigh.
