ICU patient with a complex medical history of vascular disease, diabetes, heart disease, kidney failure who presented with thigh pain, cellulitis and subcutaneous abscess.

There is extensive fat stranding starting from the inferior right pelvic region and descending inferiorly in the medial portion of the thigh which insinuates between fasciae, associated with a hypodense collection and subcutaneous air near the femoro-femoral bypass.
Case Discussion
The findings are compatible with acute necrotizing fasciitis with abscess and possible bypass infection.
