Necrotizing myositis and fasciitis

Presentation

History of bed sores and fever.

Patient Data

Age: 45 years
Gender: Male

There are multiple multiloculated peripherally enhancing subcutaneous and intramuscular collections with internal septations and multiple enormous air foci seen involving muscles groups around left pelvic girdle namely gluteal muscles and tensor fascia lata as well as all muscles groups around left thigh including abductors, hamstrings and quadriceps femoris group.

Small extension towards left groin through obturator externus muscle also seen.

Extensive subcutaneous edema noted involving lower back, pelvis and thighs with multiple skin defects noted in the left gluteal area suggestive of ulcers.

Features are suggestive of necrotizing myositis and fasciitis.

Case Discussion

Necrotizing fasciitis is a rapidly progressive inflammatory infection of the fascia, with secondary necrosis of the subcutaneous tissues.
