Neonatal bilateral pneumothorax

Presentation

2180 g. New born with deep asphyxia, APGAR 0

Patient Data

Gender: Male

In the first image there is a clear right pneumothorax and not well visible on the left.

In the rapid follow up second image bilateral pneumothorax and loss of lung volume are much clearer. There is no sign of bronchovasculer tree in the peripheral parancimal regions which is an evidence of  free air and also seen sharp edge of dens lung. There is no mediastinal shift. But the largeness of mediastenum and cardiac silhouette are compressed by the bilateral tension pneumothorax which may be lead to cardiovascular collapse. There is also subcutaneous emphysema in right upper chest wall. In the first image it is subtle and has small dimensions

Case Discussion

This new born had malpositioned endotracheal tube and rapid deterioration despite continued resuscitation. They were transferred to the local neonatal service. No follow-up imaging available.
