Neonatal hypoxic ischemic encephalopathy

Diagnosis probable

Presentation

Term neonate with low Apgar scores at delivery develops symptoms of hypoxia and seizures requiring an incubator for 15 days.

Patient Data

Age: 4 months
Gender: Female

There is bilateral diffuse cystic encephalomalacia in the white matter of both cerebral hemispheres.

Findings are more prominent in the frontoparietal lobes with marked decreased signal intensity in T1WI and FLAIR, and increased signal intensity in T2WI associated with compensatory dilatation of the 3rd ventricle, lateral ventricles and cortical sulci.

The basal ganglia, thalami, midbrain, brainstem and cerebellum are unremarkable for the patient’s age.

An arachnoid cyst is seen in the posterior fossa.

The corpus callosum is thin.
