Neonatal knee septic arthritis

Diagnosis almost certain

Presentation

Swelling of the left knee on examination. Overnight development of knee swelling. Lab - high WBC count, high CRP.

Patient Data

Age: 13 days
Gender: Male
x-ray

Metaphyseal lucent bands are noted (normal in neonates) on the frontal projection. Soft tissue swelling around the left knee. 

ultrasound

Suprapatellar complex effusion with peripheral vascularity. The collection is compressible (demonstrated in the 3rd image).

Case Discussion

Surgery the next day proved septic arthritis but without evidence of osteomyelitis.
