Headaches with bilateral uveitis in a patient with oral aphthosis.
Patient Data
Low T1 and high T2 and FLAIR multifocal lesions with areas of resctricted diffusion involving the brainstem (mainly the pons and midbrain), left thalamus, basal ganglia bilaterally (lentiform nuclei and right caudate nucleus) as well as the left optic tract, left mamillary body and tuber cinereum with surrounding vasogenic edema. The postcontrast sequences show moderate patchy enhancement of the right basal ganglia and left cerebral peduncle. No significant mass effect on the midline structures.
The dural venous sinuses are patent.
Case Discussion
In this case, the patient had oral aphthosis and bilateral uveitis in addition to the neurological manifestations: 2+2+1=5 points, indicating probable Behçet according to the international criteria for Behçet disease.
