Case contributed by Roberto Schubert
Diagnosis almost certain

Presentation

Left hemiparesis and facial hypesthesia.

Patient Data

Age: 30 years
Gender: Female
mri

Large rounded cyst in the right pontine region crossing the midline and slightly indenting the IVth ventricle. Predominantly hyperintense T2 signal within the lesion with low signal sediment with minor blooming on the B = 0 (EPI) sequence. On T1, the sediment is hyperintense and the rest of the contents isointense to brain tissue. No perifocal edema. No contrast enhancement.

Case Discussion

Stereotactic biopsy and drainage of the lesion were performed. Histologic and immunohistochemistry findings were in keeping with a neuroglial cyst with marked internal astrogliosis and protein-rich content.
