Neurological manifestation of celiac disease

Presentation

Orofacial myoclonus, imbalance, progressive confusion

Patient Data

Age: 50 years
Gender: Male

Multiple white matter lesions (T2 bright, mild enhancement, minor diffusion restriction) - gyrus rectus, periventricular, subcortical white matter

MRI (7 months later)

Prominent cerebellar and cerebral sulci inferring volume loss. Progression of existing white matter lesions. New white matter lesions: floor of third ventricle, left middle cerebellar peduncle.  Minor enhancement of corpus striatum.

 

Case Discussion

The patient unfortunately died shortly after second MRI. 

Histopathology demonstrated findings consistent with celiac disease changes in the brain.
