Neurosarcoidosis

Diagnosis almost certain

Presentation

A few weeks ago, the patient developed patchy vision with white specks seen bilaterally. They now report loss of vision in both eyes.

Patient Data

Age: 55 years
Gender: Female
mri

There is hyperintense signal involving the bilateral hypothalamus, left globus pallidus, left posterior internal capsule, bilateral optic tracts, left uncus, and right midbrain.

Corresponding nodular enhancement is observed in these regions, most prominently involving the left optic pathway, including the intracanalicular and intracranial segments, and the left uncus.

No restricted diffusion is noted to suggest an acute infarct.

No gradient blooming to suggest parenchymal hemorrhage.

Case Discussion

Imaging features favor inflammatory disease of neurosarcoidosis.

The patient was treated with prednisolone.
