Trigeminal neuralgia on the right side not responding to conservative treatment for over 7 years.
Patient Data



Cisternal segment of the right trigeminal nerve is compressed by the SCA 2 mm away from the root entry zone. Note the normal course of the left trigeminal nerve.
Incidental finding: Triangular foci of T2 and FLAIR high signal intensity with symmetric periventricular distribution around the frontal horns in keeping with epdendymitis granularis.
Case Discussion
A case of neurovascular compression syndrome in a patient with trigeminal neuralgia on the right side not responding to conservative treatment for years.
Neurovascular compression syndromes are typically caused by direct compression of the cisternal segment of a cranial nerve by a nearby crossing artery 1. Many cases are asymptomatic. Symptoms vary depending on the nerve being compressed. The most commonly affected ones are CN V, CN VII, CN VIII, CN IX and CN X.
