Non-ketotic hyperglycemic hemichorea

Presentation

Right-sided facial droop and right body numbness and dysarthria. Whole body weakness and multiple falls over the past 10 days.

Patient Data

Age: 55 years
Gender: Female

There is intrinsic T1 shortening within the left putamen, without associated susceptibility artifact. Postcontrast sequences also display uniform enhancement of the left insular cortex as well as a punctate focus of enhancement within the left frontal centrum semiovale white matter.

Case Discussion

This is a case of non-ketotic-hyperglycemic-hemichorea (also known as diabetic striatopathy). The patient had a history of severely uncontrolled diabetes mellitus (HbA1c ~18.0). Symptoms slowly improved after starting the patient on insulin.
