Non-ossifying fibroma - distal femur

Presentation

A follow up of a lesion on right femur that was incidentally noted on previous x-ray of the knee.

Patient Data

Age: 16 years
Gender: Male

A well-circumscribed lucency in the distal diaphysis of the femur with a thin zone of transition, sclerotic margins and no periosteal reaction.

Well defined (1.3 x 1.5 x 7 cm) cortically based focal lesion at the posterolateral aspect of the distal right femoral shaft with heterogenous predominately low T1 and high T2 signal and sclerotic borders. No fluid-fluid levels, no surrounding edema or extraosseous abnormality. 

Case Discussion

Non-ossifying fibromas have the following features:

  1. present in adolescents and young adults
  2. the commonest location is the femur and tibia
  3. dia-metaphyseal in location
  4. eccentric with mild expansion
  5. well defined lucent lesion with a sclerotic margin
  6. cortical thinning, but not breach.

Lesions are asymptomatic and are usually an incidental finding. Lesions often spontaneously regress.
