Non-ossifying fibroma - proximal tibia

Presentation

Left knee pain.

Patient Data

Age: 15 years
Gender: Female

x-ray

X-ray demonstrates a narrow zone of transition lucency with sharply defined sclerotic margins in the proximal of the left tibia. No periosteal reaction or cortical destruction.

MRI demonstrates an eccentric low signal (T1/T2 sequences) lesion composed of soft tissue located at the left tibia upper third anterior aspect. Cortical thinning is seen at the medial side of the lesion. No fluid-fluid levels, cortical destruction, or periosteal reaction. No surrounding edema and no extra-osseous abnormality. 

Case Discussion

Regarding the above imaging findings, non-ossifying fibroma is the most likely diagnosis. The differential diagnoses include fibrous dysplasia, fibrous cortical defect, and aneurysmal bone cyst.
