Normal thymus gland on ultrasound (infant)

Case contributed by Ian Bickle
Diagnosis almost certain

Presentation

Sepsis & Cough.

Patient Data

Age: 10 weeks
Gender: Male

Widening of the right side of the mediastinum.

Clear lungs and both costophrenic angles.

A normal thymic gland in a neonate appears as a well-defined homogeneous hypoechoic soft tissue mass in the superior mediastinum, predominantly right-side, with bulk dimensions of 3.2 x 2.4 cm.

Case Discussion

The thymus is larger in the young paediatric population and regresses with age.

As most very young children have supine projection chest radiographs, this can further contribute to the spurious appearances of a mediastinal mass.
