Diagnosis certain

Presentation

History of DVT; INR subtherapeutic on warfarin

Patient Data

Age: 25
Gender: Male

Chest radiograph

x-ray

No evidence of acute cardiopulmonary disease. Lungs are clear.

VQ study

Nuclear medicine

Homogeneous pulmonary perfusion. No moderate or large segmental or subsegmental filling defect. Ventilation images demonstrate heterogeneous radiotracer distribution with clumping of radiotracer in the central airways.

Case Discussion

Exam was performed with 30 mCi aerosolized DTPA for ventilation imaging, followed by 6 mCi intravenous MAA for perfusion imaging.

Pulmonary perfusion is normal, with no visible perfusion defect.

Ventilation images demonstrate central clumping of radiotracer, indicating turbulent airflow, a nonspecific finding.
