Obstructive ureteric calculus

Diagnosis almost certain

Presentation

Right flank pain. Full ward test (urine) revealed 1+ blood and 3+ leuks. Normal blood white cell count.

Patient Data

Age: 35 years
Gender: Female
ultrasound

Right-sided hydronephrosis and a small amount of perinephric free fluid. 3 mm echogenic focus at the right vesicoureteric junction which demonstrated 'twinkle' artifact. No ureteric jet visualized.

Unremarkable left collecting system and kidney.

Case Discussion

Ultrasound appearance and clinical presentation consistent with an obstructive right vesicoureteric junction calculus.

While most patients with acute renal colic will be investigated with a CT the diagnosis can, in many cases, be adequately confirmed on ultrasound.
