Persistent left flank pain.
Patient Data
Enlarged left kidney with moderate hydronephrosis with dilatation of the ureter up to the L3-L4 level where there is a stone (measuring 20 x 10 mm, mean density = 1320 HU), obstructing the ureteral lumen. Delayed cortical nephrogram well-visualized on excretory phase.
Normal appearance of the right kidney with no renal or ureteral stone. Absent cortical nephrogram on excretory phase.
Normal urinary bladder.

CT Urogram features of left obstructive uropathy due to ureteral stone with delayed nephrogram.
The delayed nephrogram is characterized by nephrographic enhancement in the delayed or urographic phase which persists for a prolonged period. Obstructive uropathy is considered the most common cause.
