Presentation
Headache.
Patient Data
Normal brain, brain stem, and cerebellar signal without abnormal intraparenchymal enhancement.
Prominent cerebral perivascular spaces.
Slit-like frontal horns of the lateral ventricles with partially empty sella according to patients' age. The optic nerves appear tortuous with distention of subarachnoid space around them for fundoscopic correlation to rule out papilledema and CSF pressure is recommended.
Mild bilateral meningeal thickening and enhancement are seen in the frontoparietal regions.
Occipital sinus is noted.
Homogenous enhancement of the dural sinuses and internal cerebral veins is noted.

Case Discussion
This case illustrates an incidental finding of occipital sinus draining into the right sigmoid sinus, in which some cases may be off the midline. It is worth mentioning when reporting posterior fossa pathologies that will require posterior fossa craniotomy.
