Twenty four hours of diplopia.

Patient Data

Age: 50 years
Gender: Male

Small diffusion restriction focus, hyperintense in T2 and FLAIR weighted sequences, consistent with acute ischemic infarction. The hyperintense area includes the anatomical location of the right oculomotor nucleus in the midbrain.

Case Discussion

The patient's eye examination was compatible with right oculomotor nerve palsy. Imaging shows an acute lacunar infarction involving the right oculomotor nerve nucleus.
