Presentation

2 days history of diplopia. Eye examination revealed left oculomotor nerve palsy.

Patient Data

Age: 30 years
Gender: Male
mri

A tiny focus of diffusion restriction is noted centered on the oculomotor nucleus on the left side, just anterior and to the left of the aqueduct of sylvius and within the periaqueductal grey matter. It displays bright signal on T2 and FLAIR as well. 

Case Discussion

Recent lacunar infarction is seen involving the anatomical site of the left oculomotor nerve nucleus and responsible for patient's manifestations.
