Presentation

Right cheek discharging wound.

Patient Data

Age: 25 years
Gender: Male
mri

Low T1 high T2 enhancing tract extending from the right mandibular body at the level of 46 (right first mandibular molar) where there is mandibular cortical destruction, low T1 marrow signal with enhancement. No associated fluid collection or abscess.  Asymmetric right submandibular lymph nodes.

Periapical lucencies at both 46 root apices. 

Case Discussion

Odontogenic cutaneous (orocutaneous) fistula is a rare complication of dental abscess. In this patient, an odontogenic cause was not suspected underwent a punch biopsy, which demonstrated chronic granulomatous change prior to undergoing the MRI.
