Focal circumferential fixed irregular narrowing at the middle and lower segments of the thoracic oesophagus with irregular mucosa and shouldering of the proximal end. There is delayed passage of barium and proximal oesophageal dilatation, suspicious for malignancy.

Case Discussion

The fixed circumferential oesophageal stricture giving the apple core appearance is a classic appearance of primary oesophageal carcinoma. Few patients are currently first diagnosed with oesophageal carcinoma on barium studies due to the increased role of endoscopy and CT.
