Presentation
Decrease in visual acuity bilaterally.
Patient Data
There is a large well-circumscribed extra-axial dural-based mass along the floor of the anterior cranial fossa and the olfactory grooves, measuring 6 x 5.5 x 2.8 cm. It displays an isosignal to the cortical grey matter on T1, T2, a high signal on FLAIR with vivid homogeneous enhancement, and a dural tail sign on postcontrast sequences. Surrounding vasogenic edema with a mass effect on the frontal horns, optic chiasma as well as extension to the pituitary gland which is compressed with posterior displacement of the infundibulum.
Case Discussion
MRI features of huge olfactory groove meningioma. The symptoms are usually those of the mass effect.
