Presentation

Chronic headaches with anosmia.

Patient Data

Age: 65 years
Gender: Female
mri

Large well-circumscribed lobulated extra-axial dural-based mass along the floor of the anterior cranial fossa centered on the olfactory grooves. It elicits an isosignal to the cortical grey matter on T1, T2, and FLAIR with vivid homogeneous enhancement on postcontrast sequences. There is an extension to the posterior ethmoid air cells through a bony defect of the cribriform plate. Surrounding vasogenic edema with a mass effect on the frontal horns.

Small vessel ischemic changes of the white matter.
