Presentation

Headache, difficulty speaking and left-sided hemiplegia.

Patient Data

Age: 35 years
Gender: Female

A lesion of heterogeneous attenuation located in the right frontal brain parenchyma, measuring approximately 62 x 72 x 70 mm. It contains calcifications and haemorrhage with surrounding cerebral oedema. It is compressing the frontal horns of the bilateral lateral ventricles. It erodes the inner table of the adjacent right frontal bone and shifts the midline structures of the frontal region 10 mm to the left, and partially protrudes/herniates into the right ethmoid sinus.

Left lateral ventricles are mildly dilated in keeping with hydrocephalus. Basal cisterns are partially eff.

Macroscopic:

The specimen consists of multiple samples, each with an average size of 3-5 cm, light brown in colour, and a soft cut surface.

Microscopic:

The tumour has a structure composed of cells with round nuclei, with a clear zone around the nuclei. The cell density is moderate, with few mitoses observed. Areas of calcium deposition are present.

Conclusion:

Oligodendroglioma, Grade 2.

Case Discussion

Findings on imaging and histopathology suggest an oligodendroglioma, WHO grade 2.

The patient underwent surgical resection of the tumour followed by chemoradiotherapy. Post-treatment evaluation using CT and MRI revealed no evidence of recurrence or residual tumour, with findings predominantly showing cystic encephalomalacia and postoperative fluid collections.
