Omental cake from ovarian cancer

Presentation

Presented with diffuse abdominal pain and abdominal distension 1 week before admission

Patient Data

Age: 65 years
Gender: Female

CT scanners illustrate diffuse nodular omental infiltration (omental cake) and thickening parietal. A multicystic lesion with solid components is seen above the uterus.

Additional findings:

  • dilated intrahepatic bile ducts
  • large volume of ascites
  • left lung effusion

Core sample biopsies of the omentum show tumor cells arranged in glands or scattered in fibrous tissue. The cells contain mucin vacuoles and eccentric nucleolus, floating in mucinous pools.

Case Discussion

This case shows the typical findings of ovarian carcinoma disseminating omentum with the "omental cake" sign. The cytology analysis of abdominal fluid was negative and the final diagnosis was confirmed with an omental biopsy.
