Selected images demonstrating a right saccular para-esophageal herniation containing fat material and a small amount of entrapped fluid within. 

Selected CT images confirming the para-esophageal hernia with fat tissue content. 

Case Discussion

The features are those of an omental para-esophageal hernia with a small amount of entrapped fluid within. The fluid possibly represents transudate that accumulates into the saccular herniation and can't get out to the abdominal cavity.
