Presentation

Motorcycle accident, frontal collision. Pubic symphysis diastasis upon physical examination.

Patient Data

Age: 55 years
Gender: Female
x-ray

Pelvis radiographs revealed an open book pelvic fracture as shown by the marked pubic symphysis diastasis.

The left sacroiliac joint space is slightly increased when compared to the contralateral side.

Post-operative

x-ray

Radiographs performed immediately post-op. The patient underwent open reduction and internal fixation of the pubic symphysis. Screw fixation of the left sacroiliac joint was also performed.

Case Discussion

Open book pelvic fractures are usually caused by high-energy trauma and are commonly associated with life-threatening vascular injuries.

The great degree of pubic symphysis diastasis may be distracting, but it remains important to review the whole pelvis, especially the sacroiliac joints, which are oftentimes injured as well.

In the present case, the patient did not progress to hemodynamic instability.
