Enlarged head and suspected hydrocephalus. Very large right lateral ventricle on ultrasound (not shown). MRI was recommended for further evaluation.

A very large cleft joining the sub-arachnoid space with the body of the right lateral ventricle. The cleft is lined by gray matter. Absent suptum pellucidum.
Case Discussion
The findings are consistent with open lipped schizencephaly. Absence of the septum pellucidum is a known association.
If the cleft is not lined by gray matter, then a differential diagnosis should include porencephalic cyst.
