Large well-defined mass lesion surrounds the right optic nerve and extends through the optic canal intracranially to be continuous with right sphenoid ridge/parasellar extra-axial durally based meningioma. The lesion displays isointense signal on T1 and T2 with avid enhancement after contrast administration.

Case Discussion

This long-standing untreated optic nerve sheath meningioma extended intracranially, widened the optic canal and became continuous with other durally based meningiomas. The optic nerve was atrophied and there was significant proptosis. The lesion was pathologically proved.
