Presentation

Patient presents with 3 days of painful left eye vision loss.

Patient Data

Age: 40 years
Gender: Male

MRI of the brain and orbits demonstrates swelling of the left optic nerve evidenced by hyperintensity on T2/FLAIR with effacement of the CSF within the optic nerve sheath. This is seen best on coronal T2 with fat saturation. On post contrast imaging, there is short segment avid enhancement of the optic nerve and perineural soft tissues. This is seen best on coronal T1 post contrast with fat saturation.

Case Discussion

This case represents the classic appearance of optic neuritis, an inflammatory typically demyelinating disorder which presents with vision disturbance. Diagnosis is confirmed by MRI imaging and was idiopathic in this case without other imaging findings.
