Presentation

Facial trauma, right orbital pain, and the right eye upward gaze limitation.

Patient Data

Age: 15 years
Gender: Male

Right orbital floor blow-out fracture and herniation of transected inferior rectus muscle and orbital fat within the fracture gap. Swelling of a torn segment of the muscle and partially herniated inferior oblique muscle tendon within the fracture gap is also seen.

Case Discussion

The case illustrates the non-contrast MDCT features of the herniation of the transected inferior rectus muscle and orbital fat within the fracture gap of the orbital floor blow-out fracture. The transection of the extraocular muscles in orbital blow-out fractures is a rare finding.
