Orbital floor blow-out fracture with retrobulbar and perineural emphysema
Presentation
Facial trauma and right side proptosis, limitation of movement of the right eye, and blurred vision.
Patient Data

There is a right orbital floor blow-out fracture with inferior rectus muscle and orbital fat entrapment and remarkable retrobulbar and perineural emphysema with impression on optic nerve-sheath and right eye proptosis and temporal deviation of the eye.
Case Discussion
The occurrence of retrobulbar and perineural emphysema and proptosis in orbital floor blow-out fracture is an unusual finding and is usually seen in orbital medial wall fracture that must be urgently managed due to significant compression on the optic nerve. The usual event in orbital floor blow-out fracture is inferior rectus muscle and orbital fat entrapment within the fracture gap.
