Presentation

Facial trauma and left eye pain and limitation of motion.

Patient Data

Age: 30 years
Gender: Male

The left orbital medial wall blow-out fracture with fat and medial rectus muscle hernia within fracture gap, postseptal and retrobulbar emphysema more in upper nasal side extended within upper nasal extraconal, preseptal and lacrimal gland soft tissue swelling, and ethmoid sinus hemorrhage are seen.

Case Discussion

The case illustrates the non-contrast MDCT features of orbital medial wall blow-out fracture with medial rectus muscle entrapment and retrobulbar emphysema which latter is relatively rare and must be meticulously managed because of optic nerve compression.
