Orbital paracoccidioidomycosis

Diagnosis almost certain

Presentation

Right sided vision loss with cervical lymph node enlargement and eyelid palpable node.

Patient Data

Age: 7 years
Gender: Female

Itraorbital enhancing mass lesion and two peripheral eyelid lesions with necrotic center and enhancing borders. There is also bone erosion at the lateral aspect of the right zygomatic bone.

Case Discussion

This patient was submitted to a biopsy of cervical lymph node that showed chronic granulomatous inflammatory process containing the budding yeast form of Paracoccidioides sp. with the characteristic "ship rudder wheel" ("pilot's wheel") appearance on microscopy. Ocular involvement is rare, but possible.
