Orbitotomy and Graves ophtalmopathy

Presentation

A history of drug-resistant Graves ophthalmopathy and performed orbitotomy for decompression of the optic nerve.

Patient Data

Age: 70 years
Gender: Female

Enlarged lacrimal glands with prolapse, bilateral proptosis, large orbital fat, large width of inferior, medial, and superior rectus muscles containing hypodense foci more severe in inferior rectus muscles, and preseptal swelling are seen.
Orbitotomy on both sides orbital inferior medial wall and nasal side of the floor with related orbital fat and extraocular muscles herniation within the defects are seen. Fluid and thick mucosa in most paranasal sinuses are also seen.

Case Discussion

The case illustrates the typical non-contrast MDCT features of Graves ophthalmopathy and decompression surgery for relieving the pressure of enlarged extraocular muscles on optic nerves and decreasing the grade of proptosis.
