Case contributed by Shervin Sharifkashani

Long-standing Graves ophthalmopathy with decompression orbitotomy.

Patient Data

Age: 50 years
Gender: Female

Bilateral proptosis, pronounced enlargement of all extraocular muscles, apical crowding, and bilateral orbital medial wall and floor orbitotomy are seen. Hypodense foci within enlarged extraocular muscles are seen which are mainly due to mucopolysaccharides accumulation. Empty Sella is also seen. Mucosal thickening in the right maxillary sinus and sphenoid sinus is seen.

Case Discussion

The case illustrates the typical non-contrast MDCT features of decompression orbitotomy a major treatment in full-blown Graves ophthalmopathy for preventing the compressive optic neuropathy due to enlarged extraocular muscles.
