Presentation

Right shoulder pain and weakness for the last 2 months. Clinical diagnosis - rotator cuff tear. Patient referred for sonography to check rotator cuff status.

Age: 60 years
Gender: Female

Axial view of the shoulder shows Os acromiale.

Shoulder AP oblique (standing) radiograph shows reduction in acromio-humeral distance.

' Two AC joints' - second one is seen when acromion is traced posteriorly from the acromioclavicular joint.

A tear of supraspinatus and infraspinatus tendons with 3 cm retraction falls under massive tear category. A tear of tendon results in loss of muscle belly function. This causes muscle atrophy and fatty infiltration of the infraspinatus muscle, compared to normal appearing teres minor.

An os acromiale represents an unfused accessory ossification center of the acromion of the scapula.
