Os calcaneus or anterosuperior calcaneal process fracture

28 years-old woman with history of inversion injury of her left ankle a few hours ago.

Patient Data

Age: 28
Gender: Female

Small bony fragment located between the anterosuperior calcaneal process and the cuboid bone.

Case Discussion

In the differential diagnosis, anterosuperior calcaneal process fracture due to avulsion of the bifurcate ligament or OS calcaneus secundarius were considered.

Besides the treatment we consider that this fragment had well corticated margins, small size and regular interface, supporting the diagnosis of OS calcaneus secundarius. The differentiation of these two entities may be quite challenging in some cases and MRI imaging could help us for these one.  MRI could show bone marrow edema adjacent to the fracture line.
