Presentation
History of trauma two weeks ago and acute ankle sprain for CT ankle to rule out fracture.
Patient Data

There is an incidental os calcaneus secundarius (normal variant). Otherwise unremarkable study with no evidence of bone focal lesions or healing fractures.

A red arrow is pointing to the os calcaneus secundarius.
Case Discussion
An incidental finding of os calcaneus secundarius is a normal variant that should not be mistaken as a fracture.
The os calcaneus secundarius is a well corticated bony fragment with blunt edges and regular shape, located between the calcaneus, cuboid, talus and the navicular bones.
N.B In our case backslap used because of an ankle sprain.
